INFORMATION ON GETTING THE CERTIFICATION OF DISABILITY FOR PEOPLE WITH HEARING LOSS

Instruction on getting the certification of disability

I. Procedure and papers needed to prepare:

 

1. Application for determination of disability including:

a) Application Form according to the Form No.01 attached to the Joint Circular 37/2012/TTLT-BLĐTBXH-BYT-BTC-BGDĐT

b) Copies of medical documents proving the disability: medical records, medical examination, treatment, surgery or other relevant documents (if any)

c) A copy of the Medical Assessment Council's conclusion on self-serving capacity, degree of impairment of working capacity in case of a disabled person whose conclusion of the Medical Examination Council is made before the enacting of the Decree No. 28 / 2012 / ND-CP.

d) 4 photos (3 cm x 4 cm)

 

2. Place of submission and time to return the results

a) Apply at the Ward's Department of Invalids and Social Affairs

b) Within 30 days, there will be a response and an examination by a medical evaluation board.

c) Within 05 working days from the date on which the Council's conclusion on the disability level of the disabled person is available, the President of the People's Committee of the commune shall post up and publicly announce the conclusion of the Council at People's Committee of the Commune office and issue a Certificate of Disability

 

** Note: when applying to study for your child, it is important to state that the child is with hearing loss.

*** The Department of Invalids and Social Affairs answered that there is no provision stating that children under 6 years old cannot be identified as a child with a disability.
